New Patient Document: This document provides information on what to bring to your first visit in our office.
Patient Registration: This form is filled out by all new patients and
annually by established patients to keep their records current. This can be
completed prior to your appointment in the office to save you time.
Pediatric Patient Registration: This form is filled out by all new pediatric patients and annually by established patients to keep their records current. This can be completed prior to your appointment in the office to save you time.

This office policy is specific for Dr. Krogstad's pediatric patients
Office Policy: Our Office Policy is to be reviewed and signed by all of our new and established patients.
Notice of Privacy Practices: This documents how Fresno Women's Medical
Group handles protected health information as guided by HIPAA. Copies are
posted in the office and available to our patients at anytime.
New Patient Consultation Form: This form is to be used by referring physicians to specify the needs of their patient for obstetric or gynecologic treatment.
Urogynecology Consultation Form: This form is to be completed by referring physicians to specify the services needed for their patients from our urogynecology team.
